November is National Hospice & Palliative Care Month, a time to honor the heart and soul of UpliftedCare—our extraordinary team. Every day, you bring compassion and community connection to the forefront of your work. This month, we’re shining a light on the stories that define who we are: moments that remind us why we do what we do. We’ve asked our team to reflect on their personal journeys and share what it means to uplift others through care. These reflections will be featured throughout November, celebrating the spirit of UpliftedCare and the people who make it truly special.

Heather Faber - Clinical Nurse Educator

What keeps you rooted in this work – even on the hardest days?

“What keeps me rooted in this work, even on the hardest days, is remembering why I started — to ensure that every patient and family experiences comfort, dignity, and compassion at the end of life. Hospice is sacred work, and I feel honored to support the clinicians who make that happen every day. My role as an educator allows me to pour into others so they have the knowledge, confidence, and heart to serve our community in a way that truly reflects UpliftedCare’s mission.”

Is there a moment from your work that stands out to you as especially meaningful or human?

“I’ll never forget sitting at a bedside with a family as their loved one took their final breath after working with them for several days or weeks to prepare them for this moment. The tears and gratitude come all at once. Moments like that remind me that hospice isn’t just about managing symptoms — it’s about creating space for peace and dignity at the end of life. It’s a privilege to be part of something so deeply human.”

When people express their fears of hospice to you, how do you help “redefine” their perception?

“When people express fear about hospice, I try to meet that fear with understanding. Most of the time, it comes from not knowing what hospice truly offers. I explain that hospice isn’t about giving up — it’s about shifting the focus to comfort, dignity, and meaningful moments. I remind them that our team is there to add support, not take anything away. Once people understand that hospice is about living fully until the end, you can see their shoulders drop and their breathing ease — that’s when the perception starts to change.”

How has working in hospice/palliative care changed your perspective on life, family or community?

“Hospice has changed how I view every day. I don’t take time or connection for granted anymore. It’s a constant reminder to be kind, present, and grateful — because we never know the impact we might have on someone’s life, or how quickly things can change.”

Is there a moment or person who reminded you why you chose this work?

“While I can’t name just one person, I know this work will continue to touch my own family in meaningful ways. Having the knowledge and perspective that hospice brings allows me to be a guide and source of comfort when difficult decisions or conversations arise. That’s a gift I don’t take for granted.”
