Seeking a dedicated, experienced, registered nurse with excellent assessment, critical-thinking and communication skills. Must be able to advocate for patients and their goals, make decisions independently, yet work on a team. If you have these qualities and are passionate about helping people make the most of life during precious final months, you can expand your nursing skills and experience with a mission-focused hospice. Minimum of two years experience as RN, current IL license, community home health or hospice experience preferred.

As a member of the UpliftedCares’s Interdisciplinary Team (IDT), the hospice nurse is a key person in identifying the holistic needs of the terminally ill patient and family. This is accomplished by planning, implementing and evaluating nursing care through a systematic assessment of health needs and optimum utilization of personnel. The nurse provides skilled nursing care according to the Plan of Care established by the IDT, providing symptom control and comfort with emotional support to all those involved facilitating death with dignity.

Job Type: Full-time

Pay: $63,000.00 – $78,000.00 per year

The Hospice/Palliative Care Nurse Practitioner contributes to the palliative care program and provides quality end of life care to patients and families. Clinical responsibilities include education and consultation for complex pain and symptom management, end-of-life decisions and advance care planning. Face to face hospice visits and general support to hospice program.

UpliftedCare is currently seeking a full time Social Worker with a holistic, patient and family focus. Responsibilities will include crisis intervention for our patients and families during their most difficult time. Assisting with advanced directives, psycho-social assessments, resource finding and supportive counseling. Social worker will participate in on-call responsibilities during weeknight/weekend rotation.

To provide psychosocial assessment and plan of care developed with the IDT.

To serve as a member of the IDT and participate actively in the weekly meetings.

To refer patients and families to appropriate resources in the community.

To provide supportive counseling and social services to patients and families, including anticipatory grief counseling. As requested by the Director of Social Services and Social Work Lead, complicated anticipatory grief may be referred to the Bereavement Department and worked with in conjunction with the bereavement department.

To facilitate orderly transitions of surviving clients to bereavement department services.

To maintain documentation and records consistent with UpliftedCare’s policies and procedures.

To assist in development and review of program policies and procedures.

To participate in staff, volunteer, and community training and educational programs.

To provide emotional support for members of UpliftedCare’s IDT.

Pay: $46,807.00 – $54,764.00 per year
