UpliftedCare is currently seeking a full time Licensed Social Worker with a holistic, patient and family focus. Responsibilities will include individual counseling and crisis intervention for our patients and families during their most difficult time. Will assist with advanced directives, psycho-social assessments and resource finding.
To provide psychosocial assessment and plan of care developed with the IDT.
To serve as a member of the IDT and participate actively in the weekly meetings.
To refer patients and families to appropriate resources in the community.
To provide supportive counseling and social services to patients and families, including anticipatory grief counseling. As requested by the Director of Social Services and Bereavement, complicated anticipatory grief may be referred to the Bereavement Department and worked with in conjunction with the bereavement department.
To facilitate family support groups. To assist, as requested with bereavement support groups.
To facilitate orderly transitions of surviving clients to bereavement department services.
To maintain documentation and records consistent with UpliftedCare’s policies and procedures.
To assist in development and review of program policies and procedures.
To participate in staff, volunteer, and community training and educational programs.
To provide emotional support for members of UpliftedCare’s IDT.
