$2000 sign on bonus – $1000 at 90 days and $1000 payable on 1 year service anniversary.

Seeking a dedicated, experienced, registered nurse with excellent assessment, critical-thinking and communication skills. Must be able to advocate for patients and their goals, make decisions independently, yet work on a team. If you have these qualities and are passionate about helping people make the most of life during precious final months, you can expand your nursing skills and experience with a mission-focused hospice. Minimum of two years experience as RN, current IL license, community home health or hospice experience preferred.

As a member of the UpliftedCares’s Interdisciplinary Team (IDT), the hospice nurse is a key person in identifying the holistic needs of the terminally ill patient and family. This is accomplished by planning, implementing and evaluating nursing care through a systematic assessment of health needs and optimum utilization of personnel. The nurse provides skilled nursing care according to the Plan of Care established by the IDT, providing symptom control and comfort with emotional support to all those involved facilitating death with dignity.

Job Type: Full-time

Pay: $63,000.00 – $78,000.00 per year
