Our Candlelight Memorial will include a special message from members of our staff and bereavement team, photo slideshow, music and a candlelight vigil. To honor your loved one in our photo slideshow, please submit your photo with the registration form. Photo may also be brought in or mailed to our office at 482 Main St. NW in Bourbonnais to be scanned and returned.

This program is open to anyone, even if your loved one was not a patient of UpliftedCare.
