Our Candlelight Memorial will include a special message from members
of our staff and bereavement team, photo slideshow, music and a
candlelight vigil.

To honor your loved one in our photo slideshow, please email a photo
with full name to marketing@upliftedcare.org. Photos may also be
brought in or mailed to our office at 482 Main St. NW in Bourbonnais to
be scanned and returned.

To be included in the slideshow, photos must be received by Friday, April 3, 2026.
This program is open to anyone even if your loved one was not a patient of
UpliftedCare.

To ensure adequate materials are prepared for each family,

registration for this event is required.
