HOW DOES SOMEONE ENTER UPLIFTEDCARE?
Make a Referral
One of the first questions many people have about our services is “how does someone enter UpliftedCare?” Although individuals may hear about hospice from a variety of sources, patients are usually referred to hospice services by the primary physician.
Referrals can also be made by patients, family members, friends, clergy or health care professionals. The primary physician is then contacted regarding an order for the hospice services. UpliftedCare provides services to children and adults with no age limitations.
At any time during a life-limiting illness it is appropriate to discuss all of a patient’s care options, including transitions, palliative, and hospice care. The earlier a patient is referred to our services, the greater the benefit to the patient and family.
