I hereby grant UpliftedCare permission to use my comments, statements, feedback, testimonial, photograph, video image, name, likeness, voice, and other identifying information for marketing, educational, fundraising, public relations, and promotional purposes. I understand that these materials may be used in print publications, websites, social media, videos, advertisements, presentations, and other communications developed by UpliftedCare. I acknowledge that my participation is voluntary, that I will not receive compensation for such use, and that UpliftedCare may edit my testimonial for clarity, length, or format while preserving its intended meaning. I understand that once these materials are published, UpliftedCare may not be able to control their further distribution by third parties. I release and hold harmless UpliftedCare from any claims arising from the authorized use of my testimonial and identifying information as described above.
