Advanced Care Planning provides peace of mind concerning your or your loved one’s values and wishes about healthcare and treatments. We are dedicated to carrying out your exact wishes if you are unable to do so on your own.
If you have not made an Advanced Care Plan, now is the time. By taking the time to sit down and put your Advanced Care Plan into writing and discuss it with those you love and trust, you can make sure that all of your goals and wishes are honored.
One great resource for starting the conversation and to use as a tool for you, your family, or your provider is the Healthcare Power of Attorney or HCPOA. UpliftedCare believes in helping people maintain dignity and control at every stage of life. To help community members plan for end-of-life decisions before end-of-life care is needed, UpliftedCare continues to host informal sessions explaining Healthcare Power of Attorney and its implementation.
HOW IS CARE PAID FOR
In accordance with our Mission and thanks to the generous support of our community, UpliftedCare provides services regardless of ability to pay. Hospice care from UpliftedCare is paid for by Medicare, Medicaid and most private insurance. In fact, receiving care is part of your loved one’s full entitlement of their medical insurance benefits. Medicare and Medicaid have a Hospice Benefit that covers care, supplies and medications related to the hospice diagnosis. Many private insurance plans also pay for hospice care. Medicare and Medicaid recipients are never billed for hospice services. The cost for others (those who do not qualify for Medicare, Medicaid or insurance, or those who may owe co-pays or deductibles) is determined on a sliding fee scale. Payment for Room and Board costs at the facility are not covered by your hospice benefit. Please contact the admission department at the facility where you reside for assistance with this.
