UpliftedCare is a member of the We Honor Veterans program through the National Hospice and Palliative Care Organization in collaboration with the Department of Veteran Affairs. This program strives to engage and support community hospice providers in better understanding and addressing Veterans’ unique needs.
Meet Debbie as she shares her thoughts and experiences about her father’s journey through hospice care. She speaks about how UpliftedCare provided compassionate care during his illness, continually showing respect for his service to the nation. This environment allowed her father to open up about his experiences in the service and how he was treated upon returning home. Debbie credits UpliftedCare with making her father comfortable and helping him find peace, as he realized the great appreciation for his service to the country.
Pinning & Recognition Ceremonies
We work with our facility partners or other community organizations to provide a pinning ceremony for all of the Veterans that currently reside there. We will provide the pins and recognition certificates for each Veteran along with a program that includes an invocation, brief presentation, pinning ceremony, special music and closing blessing. We invite you to offer any input you have to make the program your own.
Veteran to Veteran Volunteer Program
Our volunteer program offers the opportunity for veterans to help volunteer with our veteran patients. They can provide a presence for our patients that may include talking and reminiscing, telling stories, celebrating birthdays, assisting with pinning ceremonies, providing support for the family, just being a friend or more. Trainings are held quarterly.
Veterans Honor Wall
At the Community Grief Center a Veterans Honor Wall has been erected where Veterans can be honored and/or memorialized by purchasing a brick that will be inscribed with the veteran’s name, branch of service, rank and years of service. Call 815-939-4141 to learn more about this method of honoring a veteran loved one or friend.
Grief Support
UpliftedCare’s dedicated bereavement department offers resources for grief for both the veteran and their families. Support groups and one-on-one counseling sessions provide many excellent resources for those grieving the loss of a loved one and the unique issues that veterans face.
Educational Presentations
Our Veteran Liaison will coordinate educational presentations for facility staff and caregivers that highlight veteran specific care. Some of the topics include:
- Building a Veteran Centric Culture
- Veteran Specific Issues at the End of Life
- Service Related Clinical Issues
- Accessing Veteran Benefits
- Community Events
We work closely with local community organizations to host veteran specific community events and participate in area service projects.
